We have an experienced team that includes Registered Nurses, Licensed Vocational Nurses, Physical Therapists, Personal Assistant Liaisons, Pharmacists, Social Workers, Dieticians and Discharge Planners. They are dedicated to providing you and your family with the information necessary for you to return to your daily routines as soon as possible. Post-surgical rehabilitation, equipment, home healthcare and nursing assistance may be needed for your return home. We will coordinate this for you before you leave the hospital.
